Reading the Everything.db directly is not supported.

Is it possible to use it with Access?

Yes, with the Everything SDK.

Is it possible to manipulate it for one's own use?

No, the Everything SDK is read-only.
The closest thing Everything supports to manipulating the database is file lists.
